• A list of all medications you are currently taking
• Your dental/medical insurance information or card (if applicable)
• Payment for consultation fee (if applicable)
• A copy of the completed Patient Registration Form and Privacy Notice Form.

You may fill out Patient Registration Form and Privacy Notice Form and bring it to your first appointment. If you are experiencing any pain or have an emergency situation, call Advanced Dental NYC as soon as possible and we will try our best to see you that day. Because our dental specialists provide a variety of dental services, it is possible for certain circumstances to lengthen the estimated time frame for a procedure.
